The form below is the "Consent to Treat a Minor" form and must accompany your paperwork if treatment sought is for a person under the age of 18.
Your therapist may ask for a "Release of Information" form to be signed in cases where collaboration with other people might be helpful regarding your treatment. Other people might be your family physician, a recent therapist you were seeing, or school personnell working with your child.
The first form is "New Client Information." The second and third forms are important information for you to review, initial where necessary and sign. Please bring your forms completed and signed to your initial session.
